Membership Application - Affiliate Chapter (Form D) Download file
(submitted by Chapter) This form is for Affiliate Chapters. Over the course of the year, Form A (above), and the related fees for new members are to be collected and submitted along with Form D to the Business Office at least quarterly (ie. March, June, September and December) by the end of the month for which you wish the membership to take effect.
Not available to process online.
*Note: as of July 1, 2007, a Chapter will pay MEMBERSHIP fees to a maximum of $2,500 PER CHAPTER MEMBERSHIP YEAR. This excludes Multiple Moments subscriptions. Member details are still required ALL Chapter members to provide them with full MBC membership benefits.
